Hospice Care

Hospice care is a special type of care that focuses on comfort, dignity, and quality of life for individuals facing a life-limiting illness. Instead of seeking to cure, hospice provides physical, emotional, and spiritual support to patients and their families, helping them find peace, comfort, and meaningful moments together.

Our hospice team is made up of caring nurses, doctors, aides, social workers, chaplains, and volunteers who work together to support your loved one and your family. We help manage pain and symptoms, provide comfort, and offer emotional and spiritual support, making sure your family feels cared for every step of the way.

Pediatric Hospice Care

Pediatric hospice and palliative care provide compassionate support for children living with serious or life-limiting illnesses. The focus is on comfort, relief from symptoms, and improving quality of life, while honoring the unique needs of each child and family. Care extends beyond medical needs, offering emotional and spiritual support so families can focus on love, togetherness, and creating meaningful moments. children under 21 years old who qualify for hospice can still receive concurrent care under federal law (Section 2302 of the Affordable Care Act, 2010).

That means pediatric patients may continue to receive curative or disease-directed treatments (such as chemotherapy, dialysis, or experimental therapies) while also receiving hospice and palliative services for comfort, symptom relief, and family support.

DME and Supplies

When a loved one is on hospice, the team makes sure they have the equipment needed to be safe and comfortable at home. Hospice provides items such as a hospital bed, special mattress, oxygen, wheelchair, walker, bedside commode, shower chair, and other supplies that make daily care easier. All of this equipment is delivered, set up, and maintained by hospice at no cost to the family, so you can focus on spending time together instead of worrying about medical needs.

Our Team

Tree of Life Hospice IDG team includes:

  • Physician/Medical Director – oversees the plan of care and provides medical guidance.

  • Nurse (RN or LVN/LPN) – manages symptoms, medications, and ongoing patient care.

  • Social Worker – supports emotional, social, and practical needs, including counseling and resources.

  • Chaplain/Spiritual Counselor – offers spiritual and emotional support tailored to patient and family beliefs.

  • Certified Nursing Assistant (CNA)/Home Health Aide – assists with personal care, comfort, and daily living needs.

  • Volunteers – provide companionship, respite for families, or other supportive services.

The team meets regularly (often every 14 days) to review the patient’s condition, update the care plan, and ensure that all physical, emotional, social, and spiritual needs are addressed.

  • Neurological disorders

  • Other chronic illnesses where comfort and quality of life are the main focus.

    Hospice helps manage symptoms and provides support so patients and families can spend meaningful time together.

Schedule an evaluation

Schedule a confidential hospice evaluation for yourself or a loved one. This assessment provides a thorough review of a terminally ill patient’s physical, emotional, spiritual, and social needs to determine eligibility and develop an individualized care plan. It includes examining medical history, current symptoms, functional abilities, pain management, and the psychosocial and spiritual concerns of both the patient and their family. The purpose is to promote comfort, dignity, and quality of life by addressing all aspects of care during the end-of-life journey.
